SET Events Form

Please fill out the events form regarding your SET event.  Once submitted, a desk-pro ticket will be created and directly sent to Alice Dixon.   Questions regarding budgets, sponsorship, food and room request, please address under the comment section on the event form.    

The following does not require an event form:

Program meeting(s) and staff meetings (with the exception of Program Advisory Board Meetings).
