Support Document for WA ID and Driver’s License Application

This form is to request a Support Document for Washington (WA) ID and Driver’s License Application (a printout of your SEVIS ID page in a sealed ISSS envelope). This document is required for international students, scholars, and their dependents when applying for a WA ID Card or Driver's License. This document can establish a name, date of birth, and current WA residence address to the Washington State Department of Licensing (DOL).

You need to have your most recent I-20 or DS-2019 ready to fill out this form! 

For New Students: Please make sure your local mailing address is up-to-date in MyUW and wait until your SEVIS record is activated before making this request.

For New Scholars: Please make sure you have completed physical check-in with ISSS AND your SEVIS record is activated by UW Seattle International Scholars Operations before making this request.

Note that if you have any holds on your student account, we may not be able to assist you until all holds are removed.
