25Live Events Access Request Form

This form is to request access to reserve space on campus for events in the online 25Live reservation system. It can only be submitted by individuals with a UW NetID.

Please note the following before submitting a request:

  • Submitting a request does not guarantee that access will be granted. Each request must be reviewed by individual departmental approvers.
  • In order to submit a request, the NetID of the person needing access must be known.
  • Requests for access for a shared NetID are accepted.
  • If access is approved, the individual whom access is being requested for will be required to complete additional online trainings prior to being granted access in 25Live. Information will be sent to the person directly.

Please provide the information below for the individual who access is being requested for. If you are requesting access for yourself, please enter your own information. If you are requesting access for a supervisee or colleague, please enter their information - NOT yours.
If you are requesting access for a shared NetID, please enter the shared NetID - NOT a personal NetID.

Additional Questions

Please answer the following additional questions to assist in the review of your request.
The person being replaced will have their access to request spaces for events removed from 25Live. This includes if the person is transferring departments on campus, as they may not be approved for this access in their new unit. This is an important step for us to keep the 25Live user list accurate and up-to-date.
