Admission to and progression in the MSW program is contingent on meeting several requirements, including those noted below. Please initial each item below to demonstrate that you have read and understand these requirements.
Successful completion of a course in introductory statistics.  Please note that you must fulfill the requirement and supply unofficial documentation showing successful completion (grade of C or 2.0 or higher) prior to beginning the MSW program. You may be asked to submit a syllabus for faculty review and approval if you have not done so already.

The MSW Program requires all admitted students to obtain a background check through CastleBranch, an online service. If you accept this offer, you must initiate the background check within three weeks of receiving this offer letter. Please be aware that there is a $53.00 fee for this service. Fees are subject to change without notice. Please visit https://portal.castlebranch.com/UQ92 and place your order for ‘UQ92: Background Check’.
Submit Enrollment Confirmation Deposit of $100. This nonrefundable deposit will reserve your space in the program and will be applied to your first quarter tuition. Payments are submitted online through the application status page in your UW Graduate School application. Login to your application at https://webapps.grad.uw.edu/applForAdmiss/login.aspx.
Submit one official electronic copy (preferred method) of your transcript showing bachelor or master or higher level degree conferred to: University of Washington, Graduate Enrollment Management Services soon as possible.  For address requirements and information, please see https://grad.uw.edu/admission/understanding-the-application-process/official-transcript-requirements/.  Failure to submit an official transcript will result in a hold on your registration. If your bachelor or higher level degree was completed at the University of Washington, submission of a transcript is NOT required.
Acknowledge that you have read and understand that immunization documentation is required for all Social Work students. By signing this form you acknowledge that the submission of immunization documentation is time sensitive and must be submitted. Note that this is a requirement of the UW Health Sciences Immunization Program (HSIP)—NO EXCEPTIONS ARE MADE. Students submit immunization documentation to CastleBranch, an online service. You will visit http://portal.castlebranch.com/ux50 to set up your immunization account. You will be charged a one-time setup fee of $43. In addition, the HSIP charges an annual fee of $67 to administer this program which will be billed in quarterly installments to your student account.  Fees are subject to change without notice. If documentation is not completed a hold will be placed on your ability to register for future quarters.
